It is common to feel stuck or overwhelmed when working with clients who have complex service needs. Discussing with your supervisor can often provide the support you need. When additional support is needed, case consultations and case conferences can help identify solutions, reduce duplication of services, and improve client outcomes.
This resource contains two documents related to effective case consultations and case conferences:
- The information guide provides guidance for case managers and other refugee service providers including the difference between case consultations and case conferences, how to prepare and who should participate, and what to do during the meeting. It also contains case studies and additional resources.
- The documentation template can be customized and completed by service providers with important notes prior to, and during, the meeting. It also includes an action plan template for next steps after the meeting.
