Creating a useful community resource map requires thoughtful planning and sustained commitment. Below are the most important factors to consider before embarking on your mapping project. 

Customize Your Resource Map 

There is no one-size-fits-all approach to resource mapping. A mapping template can be a helpful starting point, but take extra care to tailor your tool to your specific community needs: 

  • Begin by clearly identifying what information you’re trying to gather and share. 
  • Reflect on your most recent referrals:  
    • What information made you confident this was a good referral? 
    • What did you learn during follow-up that would have been valuable to know beforehand?
